A nursing care plan is drawn by outlining the nursing care that will have to be provided for the treatment of patients. The plan is basically a list of actions, which the nurse would be applying to resolve, identified nursing problems through assessment. The plan assists continuing nursing care actions provisions and the care evaluation process.
The main characteristics of a quality nursing care plan can be summed up as follows:

• The main aim of plan is to minimize or solve existing problem suffered by the patient.
• The plan continues with other systematic process.
• The actions of the plan are based on future implementation.
• The plan is based on known nursing and health problems.

Nursing Care Plan Formulation

A nursing care plan is formulated with the focus of future continuing nursing care to be administered to the patient, who is suffering from some ailments or problem. It is continuing nursing process.

The first step to prepare this care plan is to collect subjective and objective data about the ailment of the patient by the nurse and arrange them in a systematic plan with guidelines. The diagnosis relates to ailment factors assessments and supports the diagnosis evidence.

After carefully determining the nursing diagnosis with Etiology, Problem, Signs and other similar symptoms system, the nurse must set the goal of expected outcome. The best way to set such outcome is by reversing the diagnosis process, by formulating, what the result would have been, if such problems do not exist. The nurse should also set a date for the expected outcome goal.

The next step after the goal is set; the plan should include nursing care that has to be administered for the achievement of that goal or recovery of the patient. The care must be specific with the specification, such as how many times it has to be followed, so that the plan can be easily read, understood and action can be followed exactly as per the plan.

The final step of the plan is to evaluate, if the set goals are met by the stipulated date. The plan shows the evidence, whether the patient has met the goal or is it still necessary that the plan should be continued, discontinue or modified.

Here it is necessary to understand that if the care plan is problem based and the patient has recovered than, the plan can be discontinued but if the problem still persists or if the plan was formulated for chronic ailments or serious problems, then the nursing care plan can be continued. Further if it is found that the care is not helping and has to be modified, and then modification action can be inserted in the plan.

The preparation of the Nursing Care Plan is solely set of goals, based on the main points of Assessment, Diagnosis, Planning, Intervention and Evaluation of clients nursing care.
