CNA License in Arizona

In order to avail CNA License in Arizona, it is necessary to completing state and OBRA-87 approved CNA Training programs offered by Arizona State Board of Nursing. The training program consists of classroom instruction and clinical trainings. The successful completion of CNA training program allows a student to appear for the CNA licensing test. The passing of licensing test gets a student listing with Arizona Nurse Aide Registry, permitting him/her eligible to work legally in long term care units, hospitals, nursing homes, long term care units and health centers.

It is also to be noted that Arizona State Board of Nursing has contracted D&S Diversified Technology to develop, regulate, administer the test and provide scoring services for Nursing Aide Testing.

The basic requirements for the Licensing test include:

  • Filling up of the 1101 and 1402 forms
  • Form must include contact details and security number.

The form must be submitted along with test fees payable by Money Orders, Cashier Checks, Facility Checks, Master card or Visa (including pre-paid Master Card or Visa). If a candidate so wish, he/she can also complete On-line registration at www.hdmaster.com. Once the approval is received from OSDH, the D&S mails or faxes the list to the candidate. Disability suffering candidate can request for ADA accommodation, which is permissible.

Licensing test consists of written and skill test. A candidate has to pass both tests with minimum 75% scoring. The passing of Licensing exam awards CNA License in Arizona and listing with Arizona Nurse Aide Registry, permitting the candidate to work legally in nursing homes, long term care units, hospitals or other health care centers.
